SUBMITTING PATIENT REVIEWS
By submitting a review to the AskaPatient database, you hereby agree that:
1) You are submitting the review on behalf of yourself, and you are a current or recent user of the drug specified on the form that you submit OR you are submitting the review on behalf of a dependent or family member, who is a current or recent user of the drug specified on the form that you submit, and that as a caregiver you are able to sufficiently consult with the patient about the effects they are experiencing from the reviewed drug in order to make a valid review on their behalf;
2) You will only submit one review per drug, unless you are updating your experience using the ‘edit rating’ option
3) You have no employment or other connections to a pharmacy-related company (such as pharmaceutical manufacturer, distributor, pharmacy, or marketer) that would influence your review in any way.
